Consultancy Programs

Assessment and Care Planning (ACFI)

This practice oriented program involves an Age Concern staff assisting your staff in assessing for care needs and developing an individualised Care Plan for one of your residents. It is about minimising documentation but maximising its effect for the individual resident.

This is not 'selling' you another Care Plan or package, but how to use yours effectively.

This program is must if any of the following occur at your facility:

  • PCA’s/AIN’s assess high care residents
  • you do not assess the cognition of residents within an hour of admission
  • if your only behaviour record is the ACFI form
  • words such as prompt, assist etc., are still stand-alone statements on your care plans
  • you are not aware of the recent review of non-verbal pain assessment tools

There is a ONE (1) day or TWO (2) day program option.

ONE (1) day program (six hours)

You arrange a suitable date and time with Age Concern Pty Ltd and then forward to us (scan or mail) a set of your admission & assessment documents. With these forms in front of her, step by step, our facilitator, Prue Mellor, will show/discuss with you:

  • how to maximise the use of each of your tools
  • the additional tools required, but not for funding
  • explain how to translate PAS and Cornell findings into care strategies
  • the issue with the Abbey pain tool, and
  • how to re-word your care plans to substantiate your funding ACFI claims.

With the distance program there is an option to break the program into three sessions each of 2 hours

Fee

  • Distance (live classroom) $880 (incl. GST) we provide you with a free headset and microphone
  • Face to Face $990 (incl. GST), plus any travel and accommodation where necessary

TWO (2) day program (12 hours)

Includes Day One (1), as above, plus Day Two (2), where we put the information together and, with you, develop a care plan for one of your residents based on your assessment data.

With the distance program there is an option to break the program into six sessions each of 2 hours

Fee

  • Distance (live classroom) $880 (incl. GST) per day.
  • Face to Face $990 (incl. GST) per day plus any travel and accommodation where necessary
