Person Centred Care Programs

If you have routines in your organisation, you probably have not completely adopted Person Centred Care practices yet. With our programs one of our facilitators comes to your facility to ‘walk’ your staff through to Person Centred Care.

Person centred care is the recognised evidenced based care for clients with dementia and the behaviours associated with dementia. We know that there is simply not enough time for you to make the required changes, so we do it for you with our preceptor style programs.

These programs are worth considering if:

  • There are still morning routines (not resident focussed)
  • There are incident reports relating to interactions between staff and residents
  • Night pads are applied about 2100 hours
  • Televisions are used in the lounge or dining room
  • Some residents have Sundowner’s syndrome or are ‘intrusive’
  • There are still two hourly turns or other ‘rounds’

The evalaution report for an organisation that purchased one of our self sustaining Person Centred Care programs, with some additional staff education, stated there was a:

  • 44% reduction of daily exposure of staff to potentially injurious behaviour from residents
  • 50% reduction in the number of residents displaying these behaviours
  • 25% reduction in behaviour related resident incident reports, and
  • The number of work cover claims has been below average in the post training period

ENQUIRE
A Three (3) Day Easy Start Program

This allows staff to experience person centred and evidence based care in their own work place, for one resident, without being overwhelmed. This is a ‘slow and steady’ start to Person Centred Care, but following our report will ensure you continue to move forward. There are up to 12 CPD hours for nurses involved.

You identify one (1) resident for assessment and care plan development. This may be a resident with ‘behaviours’ and or with a dementia.

We demonstrate to your staff how to more effectively read the assessment results to develop interventions that:

  • Show you provide person centred care
  • Incorporate behaviour management throughout the care plan and
  • Include care changes that staff may have previously resisted

ENQUIRE
An Eight (8) Day Kick Start Program

Suitable for small facilities or units of approx. 30 beds, including dementia specific units, our report will assist you to sustain the change. It also includes CPD hours for nurses and much of the Easy Start program.

For large organisations introduce the program to one unit, then you gradually ‘role it out’ to all units or sites, using your now experienced staff and our report as a guide.

With this format, we are with staff as they take the first step of person centred care, of moving away from the morning routine of waking residents for breakfast/showers. We are ‘on the floor’ with staff, across the shifts, as they experience the change and flow-one effect.

A Three (3) Week Risk Management Program

This program supports your Person Centred Care philosophy and cements practices and includes a minimum of 12 CPD hours for nurses and most of the Easy Start and Kick Start programs. Each nurse has the opportunity of receiving a minimum of 12 CPD hours.

This three-week program is useful for those organisations:

  • Commissioning a new unit
  • Really serious about risk management or
  • Transforming established units to centres of excellence

Assessment and Care Planning

This practice oriented program is about minimising documentation but maximising its effect for the individual residents and involves an Age Concern Pty Ltd staff member assisting your staff to become more efficient, effective and person centred in:

  • Data collection
  • Assessment processes
  • Person centred care plan development and evaluation

This is not 'selling' you another Care Plan or package, but how to use yours effectively, and is a must if any of the following occur at your facility:

  • PCA’s/AIN’s assist with assessments for high care residents
  • You do not assess the cognition of residents within an hour of admission
  • You care plan includes words such as repeat, assist, prompt encourage
  • Your only behaviour record is the ACFI form
  • You are not aware of the recent review of non-verbal pain assessment tools

This attracts up to six (6) CPD hours for involved staff.

One Day Format (6 Hours)

You arrange a suitable date and time with Age Concern Pty Ltd and then forward to us (scan or mail) a set of your admission & assessment documents. With these forms in front of her, step by step, our facilitator, Prue Mellor, will show/discuss with you:

  • How to maximise the use of each of your tools
  • Additional tools that may be required, but not for funding
  • How to translate PAS and Cornell findings into care strategies
  • The issue with the Abbey pain tool, and
  • How to re-word your care plans to substantiate your claims of person centred care and for funding

With the distance program there is an option to break the program into three sessions each of 2 hours.

Two Day Format (12 Hours)

Includes Day One (1), as above, plus Day Two (2) where we put the information together and, with you, develop a care plan for one of your residents based on your assessment data.

With the distance program there is an option to break the program into six sessions each of 2 hours.
